  • What is required for financial assistance?

    At least one child in the family must have active Medicaid; copy of bill or invoice; copy of birth certificates for all children; copy of picture ID for all adults; copy of social security card for entire household, proof of residence; copy of last 2 check stubs and or 2007 income tax return; statement of need.
